It varies depending on severity.  Some men only have a small breast bud to excise via a small  semi-circular incision around the areola and this can be done in the office under local anesthesia and their recovery is usually 1-2 weeks before they can return to heavy lifting and the gym.  Others require a much more extensive skin and gland removal and skin redraping (especially after weight loss) and they may have 6 weeks of rest and recovery from heavy lifting. 

It depends on the approach and severity. Some men with minimal gland presence and primarily fatty deposition over the chest can be addressed with liposuction alone via small remote “poke” access sites in the armpit crease or along the fold of the lower chest or along the boundary of the areola perimeter.  Others may require a small semi-circular incision to elevate the nipple and areola and excise directly the firm rubbery gland that cannot be liposuctioned and this leaves a well hidden (but never invisible) and imperceptible scar.  The last group may require full incisions across the chest along the inframammary fold to remove the loose hanging redundant skin and reposition the nipple and areola back up on the chest or reduce the areola which can leave well placed and hidden scars on the chest.  See your Dr. Urbinelli for more details on which approach he feels would be best for you, your goals and your anatomy.

Yes it can.  If the patient gains a lot of weight after surgery, weight can regrow breast or fatty tissue on the chest, mimicking gynecomastia again.  If the patient has an endocrine  or testosterone disorder not previously investigated or known this can regrow breast tissue.  If the patient is using a lot of THC/marijuana or anabolic steroids they may regrow their gynecomastia as well.  In general, if the patient maintains a healthy diet and weight after surgery and does not have one the aforementioned conditions above he will have long lasting results for a very very long time.
